TSH is a basic thyroid test and one of the cheaper blood tests available at private laboratories. When your doctor or you want to assess your thyroid more thoroughly, free thyroid hormones FT3 and FT4 may also be tested, sometimes along with a neck ultrasound and an endocrinologist appointment. There is less data here than for TSH alone: laboratories often show package prices only after a collection point has been selected, and package contents vary. The cheapest packages include only the TSH, FT3 and FT4 hormones, while more expensive ones also include thyroid antibodies (anti‑TPO and anti‑TG).

Private TSH test or testing through the NFZ?

A TSH test through the NFZ is free, but it requires a referral, usually from a GP. A private test typically costs PLN 37 ‑ 41, but you do not need an appointment and will receive the result quickly. The choice therefore depends mainly on how much time a doctor’s appointment would take and whether you need to see a doctor anyway.

In practice, many people have their first TSH test done privately because it is inexpensive and readily available, then take the result to their GP. For ongoing thyroid treatment, follow-up tests are usually ordered by the doctor managing the treatment and are then free. A doctor will order an FT3 and FT4 package through the NFZ if they consider it necessary. You can order it yourself privately, but it is still worth discussing the result with a specialist.

For comparison, a TSH test (PLN 41) costs only slightly more at a laboratory than a full blood count (PLN 25), while a thyroid package costs about as much as several individual TSH tests. These are still modest amounts compared with an endocrinologist appointment or an ultrasound scan.

Do you need to fast before TSH, FT3 and FT4 tests?

Laboratories usually recommend having TSH, FT3 and FT4 tests in the morning, preferably while fasting or after a light breakfast, although TSH itself is not greatly affected by food. Consistency is more important than meals: follow-up tests are best performed at a similar time of day because TSH levels change throughout the day.

If the TSH test has been ordered together with other tests, such as glucose or a lipid profile, you will need to fast because of those additional measurements. The laboratory provides detailed preparation instructions when you order the test.

Should a TSH test be taken while fasting or after taking your tablet?

Laboratories and doctors usually ask people taking thyroid medication (levothyroxine) to have their blood drawn before their morning tablet and to take the medication afterwards. This helps prevent the latest dose from distorting the result and makes subsequent measurements comparable.

What should you avoid before a TSH test?

  3. Keep follow-up tests at a consistent time. Results obtained at different times of day are harder to compare.

TSH test results are usually available within a few hours to 1 working day. A package including FT3 and FT4 may be ready within the same period. Most laboratory chains make results available online or through an app. Packages including antibody tests may take an extra day or two.

A result alone is not a diagnosis. Reference ranges vary between laboratories, and interpretation depends on age, medication, pregnancy and symptoms, so discuss the result with your GP or endocrinologist.
