What affects the price
- whether the application concerns an ordinary administrative surname change (PLN 37) or reverting to a pre-marriage surname within 3 months of divorce (PLN 11)
- whether an authorised representative is handling the case (an additional PLN 17 stamp duty)
- outcome of the procedure: approval (PLN 37) or refusal (PLN 10)
The fee is uniform nationwide and set by the Stamp Duty Act; the same rate of PLN 37 appears in the Public Information Bulletins of many different authorities.

How much does it cost to change your surname at a government office?
The stamp duty for a decision approving a change of first name or surname is PLN 37; if acting through a representative, an additional stamp duty of PLN 17 is charged for the power of attorney.
Where do you need to update your details after changing your surname?
An identity card, vehicle documents and details held by other authorities and institutions must be updated separately; the civil registry office's decision does not update them automatically.
